Case management is extremely beneficial when an individual or family is trying to navigate providers and resources to help themselves or their loved ones heal. It can be chaotic and extremely difficult to find the proper resources and more importantly to ensure things are continuously going smoothly with the entire team on the same page. Care gets fragmented and it quickly develops into a lot of time, and energy, and money wasted on just trying to keep the status quo let alone actually heal.
First, if the client doesn’t yet have the providers and resources they need, we connect them to the people that are the best fit for them. We also help connect them to community resources that can help with the individuals' specific needs. Once the clients' clinical needs and outside resources are in place we act as a central point of contact to help everyone stay on the same page regarding the care being provided.
We are in touch with all providers as needed. We meet with the individual doing the healing work weekly to go over what’s working and what’s not and where they could use extra support. We also meet with the family or caregivers weekly to listen and act on their needs. This makes everyone’s jobs easier but more importantly everyone can focus on doing the extremely important work they are doing to help the individual move forward. The therapist can focus on real therapy whether that’s trauma work or other modalities. If there is a psychiatrist they can focus on the medicine. And the family can focus on being there for their loved ones without having to be overloaded juggling all their care.
Because we are actively up to date on the day-to-day progress as well as any issues or areas that need adjusting, we act as the providers and families point of contact in real time instead of broken telephone causing care to drag along.
